• Enrollment * fee: $695
• Enrollment * fee: $500 - reduced enrollment fee for medical technicians/residents/trainees

• Daily Enrollment fee: $150 per day

* Enrollment fee includes daily continental breakfast, Mexican Fiesta buffet, course syllabus and materials and a $75 non-refundable administrative fee. Notice may be given by calling (801) 581-5873 and must be followed by a written request for cancellation. Allow three weeks for processing of any refund.
